Waiver and Release of Liability

Important Notice: Participation in football involves inherent risks, including but not limited to physical injury, concussions, and other injuries. This Release of Liability and Waiver Form is a legally binding agreement that releases CS-Athletics, its organizers, coaches, staff, and affiliated parties from liability for any injuries or damages sustained during club activites.

1. Assumption of Risk: I, the undersigned parent/guardian, understand and acknowledge that tackle football is a physically demanding contact sport that involves inherent risks including but not limited to sprains fractures, concussions, and other serious injuries. While the club takes safety precautions for safe tackling practices, there is no guarantee of injury prevention. My child is voluntarily participating in this club and assumes all risks associated with participation.

2. Release of Liability and Indemnification: I, on behalf of myself, my child, my heirs, assigns, and legal representatives, agree to release, waive, and discharge: CS-Athletics, its directors, coaches, trainers, volunteers, and staff; Any sponsors, partnering organizations, or facility owners; from any and all liability, claims, or demands for injuries, damages or losses incurred by my child during participation. I future agree to indemnify and hold harmless parties from any claims or expenses (including legal fees) that arise from my child's participation.

3. Medical Consent and Insurance Acknowledgement: I certify that the athlete(s) is/are physically fit and able to participate in club activities. I understand that it is my responsibility to inform CS-Athletics of any medical conditions that may affect participation.

   In the event of illness or injury, I authorize CS-Athletics and its representatives to obtain emergency medical treatment for the athlete(s), including transportation to a medical facility if deemed necessary.

I understand that:

• I am responsible for all medical expenses incurred.

• I am responsible to obtain an AAU Membership for my athlete using the CS-Athletics club code.

• It is my responsibility to maintain adequate health and accident insurance coverage.

4. Photo and Media Release: I authorize CS-Athletics to use photographs and/or video recordings of my child for promotional purposes, including social media, website content, and marketing materials.

Parent/Guardian Acknowledgment: I have read carefully and fully understand this Release of Liability and Waiver Form. By Selecting 'I agree' and submitting this form, I certify that I am the parent or legal guardian of the athlete named. I confirm that I have read and understand the waiver and the acknowledgements and voluntarily agree to all terms. I understand this submission constitutes my electronic signature.
